Usage flow
1) ① Please complete the "Application Form for Admission" and send it by email to the Labor Management Section of the General Affairs Division at Hokkaido University Hospital by the deadline.

2) You will receive a notification from the nursery school regarding an interview. Please submit the "Required Documents for Temporary Childcare Application" (item ②) to the interview.
3) On the day of temporary childcare, please submit the "Request to Parents" form (③).
